Evolutionary Healthcare is seeking compassionate Nurse Practitioners (NPs) and Physician Assistants (PAs) to join our innovative Osteopathic Medicine team serving skilled nursing and post-acute care facilities throughout Central Florida.
This unique role is ideal for providers who enjoy hands-on patient care, musculoskeletal medicine, pain management, and working closely with Osteopathic Physicians to improve mobility, function, and quality of life. While OMT is performed by our physicians, you will play a vital role in identifying appropriate patients, managing their medical care, and supporting comprehensive treatment plans.

What You'll Do
As an OMT Support Provider, you will:
- Provide comprehensive primary care services in skilled nursing and post-acute care facilities.
- Evaluate patients experiencing musculoskeletal pain, mobility limitations, balance disorders, and functional decline.
- Identify patients who may benefit from Osteopathic Manipulative Treatment and collaborate with our OMT physicians on treatment planning.
- Manage acute and chronic medical conditions that affect rehabilitation and recovery.
- Perform admission evaluations, routine follow-up visits, and post-hospital assessments.
- Coordinate care with physicians, therapists, nursing staff, and facility leadership.
- Monitor patient progress and help optimize functional outcomes.
- Educate patients and families regarding treatment plans and preventive care.
- Maintain timely, accurate documentation using electronic health records.
Ideal Candidate
We're looking for providers who:
- Enjoy working with geriatric and post-acute patients
- Have an interest in musculoskeletal medicine and functional improvement

- Thrive in relationship-based patient care
Qualifications
- Licensed Nurse Practitioner or Physician Assistant in Florida (or ability to obtain)
- National Board Certification
- DEA license (or ability to obtain)
- Experience in Internal Medicine, Family Medicine, Geriatrics, Hospital Medicine, or Post-Acute Care preferred
- Skilled Nursing experience is a plus but not required
- Strong communication and clinical decision-making skills
- Passion for patient-centered care

Evolutionary Healthcare began with a simple yet profound observation: the gap in care between hospitals and skilled nursing facilities was glaring. As dedicated hospitalists, we frequently admitted patients from nursing homes, and it didn’t take long for us to realize that something needed to change.

A Disconnect in Care: Upon reviewing the medical records and charts of these patients, we were struck by the stark difference in the quality of care they received in nursing homes compared to hospitals. Physicians rarely visited their patients in these facilities, and even when they did, they often struggled to remember them or truly understand their unique medical needs. Vital lab results were not consistently reviewed, and the overall management of medical conditions left much to be desired.
A Call for Accountability: Witnessing these shortcomings, we felt a moral and professional obligation to act. We recognized that skilled nursing facilities deserve a higher standard of care and accountability. It was time to bridge the gap between hospital-level care and the specialized needs of patients residing in these facilities.
